Quality Assurance RN Manager - Home Health

The Manager of Quality Management and Education (Mgr QM/ED) will oversee the development, implementation, and evaluation of the agency’s quality improvement, staff education program’s, which includes all departments, to identify trends, prioritize and recommend improvements, develop policies, and ensure compliance with State and Federal Regulations and Accrediting Agencies. The Manager of Quality Management and Education investigates incidents, patient complaints, patient care issues, and other issues, identifies reports trends and implements plan of corrections. Clinical Data Entry and Orders Coordinator.

The Manager of Quality and Education is responsible for overseeing the agency’s internal quality control activities. The position also includes oversight of the Home Health Aide Supervisor/Intake Coordinator, Clinical Data Entry and Orders Coordinator. The position reports to the Vice President.

Qualifications:

Must be a home care Registered Nurse (RN) who has experience in doing QA primary responsibility will be chart reviewing in a quality assurance (QA) manner. Recent experience with QA in the home health care setting as an RN is required.

  • Bachelor’s degree in Nursing or the equivalent
  • Minimum of three to five years nursing experience, including two years of home healthcare experience within the past five years
  • Prior ICD-9/10 coding experience
  • R.N. License
  • Working knowledge of Medicare home health requirements
  • Analytically inclined and detailed oriented to ensure proper and accurate coding for reimbursement


Essential Responsibilities:

  • Plans, directs and implements the agency’s internal quality control activities
  • Plans, directs and implements the agency’s orientation and staff education development programs
  • Supervises the agency’s Intake Coordinator
  • Oversees the agency’s Intake and Referral Process
  • Supervises the agency’s Utilization Review Staff, Intake Supervisor / Orientation Manager and oversees the agency’s management of on-site medications, clinical record audit including peer review
  • Supervises the agency’s Clinical Data Entry Personnel and Orders Coordinator
  • Performs and provides a quarterly summary report on Quality Indicators that reflect clinical record review audit results, Infection Statistics, Patient Complaints and/or Satisfaction, incident reporting and OBQI/OBQM monitoring and review ongoing Quality Improvement projects
  • Participates in self evaluation and professional development
  • Participate in Administration On-Call program
  • Maintains reviews and reports trends and plan of correction for risk management including agency on-call and complaint logs, infection control, incidents, medication errors, wound care, NHNF, and high risk patients
  • Develops and maintains the agency’s Orientation

Greater New Haven Home Care agency is seeking a Clinical Supervisor who will be responsible for maintaining quality and continuity of clinical service in accordance with agency objectives, standards, and policies.  EOE

The RN Supervisor of Clinical Services must possess outstanding supervisory skills, presenting a professional attitude and appearance at all times, maintaining confidentiality of all client and employee information. The Clinical Supervisor  must possess a high level of creativity and good judgment.


Essential Responsibilities of the Clinical Supervisor:
       

  • Assist in developing and interpreting personnel policies for clinical services.
  • Participates in screening and interviewing new employees.
  • Performs periodic evaluation of new and current employees.
  • Participates in disciplining and terminating employees as needed.
  • Monitors documentation of utilization of clinical services.
  • Reviews all materials/data sent to the fiscal intermediary relating to clinical matter.
  • Reviews patient records on ongoing basis to determine duration/frequency of clinical service and appropriateness.
  • Communicates appropriate information to staff regarding daily operations and agency goals/objectives.  
  • Coordinates clinical services with other agency staff.
  • Acts as a liaison between staff, administration and referring agencies. 
  • Attends case conferences.
  • Acts as a resource person in the field of psychiatric nursing to the agency.  
  • Participates in the development and interpretation of agency policies, procedures, objectives and standards of care.


Requirements

The requirements for the Clinical Nurse Supervisor position as set forth by the Connecticut Department of Public Health are:

  • Master's Degree with  minimum of 1 year full-time clinical experience in a home health agency or a B.S. in Nursing and 3 years full-time experience in nursing, at least 1 of which was in a home health agency.
  • Current nursing license in the State of Connecticut.

Registered Nurse / RN - Continuum Home Health Inc.

Duties of the Home Health RN:

The Primary Care Nurse is responsible to provide within a family centered approach, skilled nursing or therapeutic services in the home and other appropriate settings in accordance with agency policies and procedures.

  • The Primary Care Nurse promotes, maintains, and restores the health of individuals and the community though appropriate teaching, counseling, case finding, and rehabilitative measures.
  • Registered Nurse plans and provides skilled nursing service to the clients in the home.
  • Registered Nurse supervises Home Health Aides providing direct patient care.
  • Registered Nurse will plan and manage assigned caseload.
  • Registered Nurse models appropriate and professional appearance and behavior.
  • Registered Nurse shows respect for the client's privacy and protects the client's personal property.
  • Registered Nurse performs all client care related activities according to Continuum Home Health, Inc.

Requirements:

  • Registered Professional Nurse with a current Connecticut license.
  • 2-5 years home care nursing experience and/or general nursing experience preferred, but not required.
  • Must hold a valid Connecticut driver's license.

 

Home Health Aide - Continuum Home Health Inc.

Duties of the Home Health Aide:

The Home Health Aide provides personal and hygiene care to ill or handicapped patients in the home or other appropriate setting.

The Home Health Aide follows the nursing care plan and reports to the primary nurse and/or the nurse designated as the Home Health Aide Supervisor.

Requirements:

  • Successful completion of a training and competency program for HHAs approved by the State of Connecticut Department of Health Services.
  • Successful completion of a nurse assistant or nurse's aide training course approved by the State of Connecticut Department of Health Services and completion of competency evaluation.
  • A practical or registered nursing student who has successfully completed not less than 75 hours of theory and clinical instruction and a competency evaluation.

Physical & Mental Effort:

  • Medium work exerting 20 to 50 pounds of force occasionally (up to 1/3 of the time), and /or 10 to 25 pounds of force frequently (1/3 to 2/3) of the time, and/or greater than negligible force up to 10 pounds consistently.
  • This position requires daily driving an automobile varied distances to client homes and the ability to enter/exit an automobile numerous times daily.  Must have the ability to enter buildings using varied levels of stairs in order to provide service to clients.
  • The Home Health Aide presents a professional attitude and appearance at all times and maintains confidentiality of all client information.
