Greater New Haven Home Care agency is seeking a Clinical Supervisor who will be responsible for maintaining quality and continuity of clinical service in accordance with agency objectives, standards, and policies. EOE
The RN Supervisor of Clinical Services must possess outstanding supervisory skills, presenting a professional attitude and appearance at all times, maintaining confidentiality of all client and employee information. The Clinical Supervisor must possess a high level of creativity and good judgment.
Essential Responsibilities of the Clinical Supervisor:
- Assist in developing and interpreting personnel policies for clinical services.
- Participates in screening and interviewing new employees.
- Performs periodic evaluation of new and current employees.
- Participates in disciplining and terminating employees as needed.
- Monitors documentation of utilization of clinical services.
- Reviews all materials/data sent to the fiscal intermediary relating to clinical matter.
- Reviews patient records on ongoing basis to determine duration/frequency of clinical service and appropriateness.
- Communicates appropriate information to staff regarding daily operations and agency goals/objectives.
- Coordinates clinical services with other agency staff.
- Acts as a liaison between staff, administration and referring agencies.
- Attends case conferences.
- Acts as a resource person in the field of psychiatric nursing to the agency.
- Participates in the development and interpretation of agency policies, procedures, objectives and standards of care.
Requirements
The requirements for the Clinical Nurse Supervisor position as set forth by the Connecticut Department of Public Health are:
- Master's Degree with minimum of 1 year full-time clinical experience in a home health agency or a B.S. in Nursing and 3 years full-time experience in nursing, at least 1 of which was in a home health agency.
- Current nursing license in the State of Connecticut.

Duties of the Home Health RN:
The Primary Care Nurse is responsible to provide within a family centered approach, skilled nursing or therapeutic services in the home and other appropriate settings in accordance with agency policies and procedures.
- The Primary Care Nurse promotes, maintains, and restores the health of individuals and the community though appropriate teaching, counseling, case finding, and rehabilitative measures.
- Registered Nurse plans and provides skilled nursing service to the clients in the home.
- Registered Nurse supervises Home Health Aides providing direct patient care.
- Registered Nurse will plan and manage assigned caseload.
- Registered Nurse models appropriate and professional appearance and behavior.
- Registered Nurse shows respect for the client's privacy and protects the client's personal property.
- Registered Nurse performs all client care related activities according to Continuum Home Health, Inc.
Requirements:
- Registered Professional Nurse with a current Connecticut license.
- 2-5 years home care nursing experience and/or general nursing experience preferred, but not required.
- Must hold a valid Connecticut driver's license.
