1. Who can call to set up a home health care evaluation?

Anyone! The patient can call Continuum Home Health, or a family member, caregiver, case manager or clinician can call on the patient’s behalf. Anyone who is concerned for the patient’s health and well-being can call Continuum Home Health.

We will do all the follow-up, including getting the proper orders, handling insurance authorization, contacting the doctor, and coordinating services. We even have a financial advisor who can assist patients with their paperwork, free of charge.

2. What information will I need about the patient and his or her condition?

Our intake staff will gather the patient's primary information, including:

  • patient's name
  • Social Security number
  • date of birth
  • doctor and contact Information
  • medication, if known; if you don't have this information, we ¬†will get it when we meet with the patient or speak with the doctor

3. How soon will a case manager or nurse be available?

Depending on the situation, we can usually get someone out to the patient’s home within 24 hours. We have more than 50 full-time staff members and we understand how to deal with emergencies and crisis situations. We will assess your needs and put together a plan of care which can be acted upon immediately. Our nurses work morning, afternoon and evening shifts to accommodate patients 365 days a year.

4. How does the process work?

When you call Continuum Home Health, a home health specialist will listen to you and answer your questions. This conversation is followed by a personal assessment in the privacy and comfort of your own home. You will find out what to expect given your specific condition. Our counselors will address your issues and concerns and explain how our nurses, therapists and home health programs can assist you. In close cooperation with you and your family, we will build a personalized care plan, subject to your doctor’s approval, that meets your healthcare needs.
